Chesapeake, located in the beautiful Coastal Virginia region, is an outdoor lover's paradise, offering hiking trails for all experience levels and more than 20 waterways to explore. We are seeking a dedicated Home Health Physical Therapist to join our team.

The Home Health Physical Therapist plans and administers medically prescribed physical therapy treatments for patients suffering from injuries, or muscle, nerve, joint, and bone diseases, in an effort to restore function, relieve pain, and prevent disability. Key responsibilities include:

• Providing education and information to patients and caregivers.
• Participating in required discharge planning and completing all required documentation according to policy.
• Directing patient participation in selected tasks to restore, reinforce, and enhance performance.

Schedule: 8:00 AM to 5:00 PM, Monday through Friday. Weekend coverage for Ortho cases only, with PT every 4th or 5th weekend. The unit information includes mileage reimbursed at IRS rate and an expectation of 6 to 7 weighted visits per day.

Minimum Requirements:

• 1 year experience required - Home Health experience preferred.
• Unit is open to first-time travelers!
• MatrixCare or Epic experience preferred, not required.
• VA PT State License Required - CAN BE PENDING!!!!
• CPR Required.
