We are seeking a dedicated Occupational Therapist (OT) for Home Health Therapy/Rehabilitation in San Ramon, CA. This is a fantastic opportunity to join a leading healthcare provider and make a difference in patients' lives through personalized rehabilitation services.

The Occupational Therapist will primarily work in home health settings, covering areas such as Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City, Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont. The position requires a commitment to patient care and effective communication.

Key Responsibilities:

• Provide one-on-one therapy to patients in their homes.
• Manage a caseload of 5-6 patients per day.
• Ensure timely communication and documentation of patient progress.
• Utilize provided documentation tools and return them as required.
• Maintain a high standard of patient care and safety.

Requirements:

• Must have a valid California Occupational Therapist license.
• 1 year of Home Health experience preferred.
• BLS certification required.
• Must have reliable transportation for patient visits.

Additional Information:

• Shift: Day 5x8-Hour (08:00 - 16:30)
• Guaranteed hours: 36 hours weekly (not applicable in the first 2 weeks for ramp-up, holidays, and time off).
• Free parking and mileage reimbursement at IRS standard rates.
• First-timers are accepted, and locals are welcome to apply.
• No on-call duty is required.
