Key Responsibilities:

Provide case management services to patients, ensuring they receive appropriate care and resources.
Collaborate with healthcare teams to develop and implement care plans.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families about health management and available resources.

Requirements:

Must have at least 2 years of acute care experience.
First-timers accepted with clinical approval.
Weekend requirements: Must work every other weekend.
Willingness to float between like units based on need.
All time-off requests (RTO) must be included at the time of submittal.

Charting: Epic
Free parking on the first day; assignment parking provided during orientation.
Local candidates are accepted.
