Join a leading acute-care hospital as a Case Manager Registered Nurse in Los Angeles, CA. This contract position offers a unique opportunity for RNs with a passion for case management and patient advocacy. With a focus on care coordination and discharge planning, you will play a vital role in ensuring patients receive the best possible care during their hospital stay.

Key Responsibilities:

• Perform care coordination and discharge planning for patients.
• Work collaboratively with multidisciplinary teams to optimize patient outcomes.
• Manage a patient caseload with a ratio of 1:20.
• Participate in weekend rotations as needed to meet departmental needs.

Requirements:

• Minimum of 2 years of acute care hospital case management experience.
• Current California state RN license (pending licenses accepted, but must be licensed by start date).
• First-time travelers are welcome; clinical screening will be conducted.
• No more than 7 days of requested time off (RTO) during the contract period.
