Join a leading healthcare organization as a Case Manager Registered Nurse in Petaluma, CA. This contract position offers an exciting opportunity for RNs with experience in case management, particularly those familiar with EPIC systems.

As a Case Manager RN, you will float between Petaluma and Healdsburg, which are approximately 30 minutes apart. Your schedule may vary, with potential assignments of 3 days at one location and 2 days at another, or a full week at a single site depending on staffing needs and PTO. This role includes every other weekend.

Key Responsibilities:

Utilize EPIC for patient management and documentation.
Coordinate care plans and communicate effectively with patients and healthcare teams.
Float between facilities as required, adapting to different environments and teams.
Ensure compliance with healthcare regulations and standards.

Requirements:

Registered Nurse (RN) license in California.
Experience in case management.
EPIC experience is a must.
