Join a leading healthcare organization as a Case Manager Registered Nurse in Clinton, MD. We are seeking a dedicated RN with a passion for patient care and a strong background in case management. Key Responsibilities:

• Manage a caseload of up to 20 patients per day, focusing on discharge planning and care coordination.
• Collaborate with interdisciplinary teams to ensure quality patient outcomes.
• Float to medical units, ICU/CCU, and Maternal Child as needed.
• Participate in every other weekend and holiday rotations.

Qualifications:

• Must have at least 2 years of nursing experience, with travel experience preferred.
• Experience in acute hospital settings (ICU, MS, SDU/PCU/IMC/Obs, Women's Services) is required.
• Certifications: BLS and a valid Maryland nursing license are mandatory.
• Social Workers (LMSW, MSW, or LCSW) with 2+ years of experience will also be considered.

All employees must have the flu vaccine as per facility requirements. This position is ideal for RNs who are looking to enhance their skills in case management while enjoying a structured work schedule.
