**Key Responsibilities:**

• Provide case management services to patients, ensuring continuity of care.
• Collaborate with healthcare teams to develop and implement patient care plans.
• Conduct assessments and evaluations to determine patient needs.
• Facilitate communication between patients, families, and healthcare providers.

**Requirements:**

• 2 years of Case Manager experience required.
• Current RN License required; Nursys must be on profile.
• BLS certification required.
• Degree accreditation or 5 years of experience required.
• Willingness to float to other sister campuses as needed.

**Additional Information:**

• All RTO must be submitted with application; no approvals after submission.
• Demographic information (Address, email, full DOB, full SS, RN license number) must be loaded at the time of submission.
• Culture is very important; travelers are expected to integrate with core staff.
• Hard document deadline is the Monday prior to start; no exceptions for pending documents.
