Position Overview: As a Case Manager RN, you will be responsible for care coordination, discharge planning, and disease management. You will work within a multidisciplinary team to ensure optimal patient outcomes, adhering to CMS guidelines and HIPAA regulations.

Key Responsibilities:

• Acute Hospital MS SDU/PCU/IMC/Obs Surgical Services
• Conduct needs assessments and order DME
• Develop and implement plans of care
• Ensure compliance with The Joint Commission standards and National Safety Goals

Requirements:

• Minimum 2 years of experience required
• Business casual attire required; scrubs not necessary
• RTO must be included in the submittal; no RTO the first two weeks of the assignment

Holidays and every other weekend are required, with a preference for working every weekend. Please ensure that all requested time off is confirmed for the full 13-week contract, as changes can only be made to cancel time requested.
