Join a leading acute-care hospital as an RN Case Manager in San Antonio, TX. This contract position offers a unique opportunity to coordinate and facilitate patient care through effective collaboration with the Interdisciplinary Care Transitions (ICT) team. You will follow patients throughout the continuum of care, ensuring the optimum utilization of resources and compliance with external review agencies.

Your role will involve providing ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. Enhance the quality of patient management and satisfaction while promoting continuity of care and cost-effectiveness through the integration of functions of case management, utilization review, and discharge planning.

• BLS certification (must be attached to submission packet; must be through the American Heart Association)
• Minimum 2 years recent RN Case Management experience, with experience in a LTAC or STACH setting, MCG, discharge planning across acute care settings, and strong IDT collaboration
• Texas RN or multi-state RN license (must be attached to resume)
