Join a leading healthcare provider as a Physical Therapist specializing in Rehabilitation in Kingman, AZ. This contract position offers a unique opportunity to make a difference in patients' lives while enjoying a flexible schedule and competitive pay.

**Important Note:** This position adheres to a 100-mile radius rule, and we are unable to proceed with clinicians who live within 100 miles of the facility. While OASIS experience is not required, home health experience is preferred.

Shift: This role involves a Day 4x10-Hour shift from 07:00 to 17:30, totaling 10 hours per day.

Experience Requirements: A minimum of 1 year of experience is required. First-time travelers are welcome to apply.

Certifications Required: Candidates must possess the following certifications at the time of submission:

• Certified Physical Therapist in AZ
• BLS Certification
• Level One Fingerprint Card

On-Call Requirements: No on-call duties are required for this position.

Home Health PT/Outpatient PT Experience: Experience in home health and outpatient physical therapy is essential, with familiarity in treating the following diagnoses:

• CVA (Cerebrovascular Accident)
• HF (Heart Failure)
• DMII (Diabetes Mellitus Type II)
• S/P Amputation
• Parkinson's Disease
• Paraplegia
• Dementia

This facility will reimburse mileage at the IRS standard rate, which is currently $0.76/mile. Expect to drive anywhere from 30 to 75 miles per day, with the possibility of driving up to 150 miles in one day.
