Unit Information: You will be working in the Case Management department, which includes a unit-based RN Case Manager and Social Workers. The patient ratio is approximately 1 RN Case Manager to 20-25 patients, ensuring manageable workloads and quality care.

Key Responsibilities:

• Provide comprehensive case management services to patients.
• Coordinate care with interdisciplinary teams to ensure optimal patient outcomes.
• Utilize Meditech for electronic medical records management.
• Work collaboratively with social workers and other healthcare professionals.

Qualifications:

• Must Have: Hospital Case Management Experience.
• Preferred: Experience with Interqual and Meditech.
• Strong attention to detail, time management, and multi-tasking skills.

As a Travel RN Case Manager, you will be expected to work every other weekend and holidays in rotation with hospital employees, adhering to facility policies.
