Seven Healthcare is seeking a motivated Physical Therapist – Home Health to join a reputable facility in Danbury, Connecticut for a travel assignment.
This assignment offers a competitive weekly pay of $3,400, based on experience and qualifications. The successful candidate will provide home health physical therapy services on 5x8 day shifts, working 40 hours per week, with an ASAP start date. This role supports patients in their home environment through individualized treatment plans focused on mobility, function, safety, and independence.

Key Responsibilities of the Home Health Physical Therapist
• Conduct in-home patient evaluations and develop individualized therapy plans
• Deliver therapeutic exercises and interventions to restore mobility and functional independence
• Assess home safety and recommend appropriate modifications or assistive devices
• Educate patients and caregivers on treatment plans and home exercise programs
• Track progress, update care plans, and coordinate services with the broader care team
• Maintain accurate documentation and comply with home health standards and regulations
• Communicate effectively with physicians, case managers, and interdisciplinary teams
Requirements for a Home Health Physical Therapist
• Active Connecticut PT license or eligibility to obtain
• Home health experience preferred
• BLS certification required
• Strong autonomy, time management, and patient education skills
