Key Responsibilities of the Physical Therapist – Home Health:
Evaluate patients in their homes to assess mobility, strength, and functional limitations
Develop and implement individualized treatment plans to improve independence and quality of life
Provide therapeutic exercises, manual therapy, and mobility training
Educate patients and caregivers on home exercise programs and safety strategies
Monitor patient progress and adjust care plans as needed
Document patient care, progress, and outcomes accurately and timely
Coordinate care with physicians, nurses, and interdisciplinary healthcare teams
Ensure compliance with home health regulations and safety standards
Requirements:
Active Physical Therapist (PT) license in South Carolina
Minimum 1–2 years of recent physical therapy experience, preferably in home health
Current BLS certification
Valid driver’s license and reliable transportation
Strong clinical, organizational, and time-management skills
Excellent communication and patient education abilities
