Seven Healthcare is pleased to offer a travel opportunity for an RN – Case Manager (Home Health) in Mebane, NC. This assignment consists of 8-hour day shifts, five days per week, totaling 40 hours, with a weekly pay of $2,600.

Key Responsibilities of the RN – Case Manager (Home Health):

Conduct comprehensive in-home patient assessments

Develop, implement, and oversee individualized care plans

Coordinate care with physicians, therapists, social workers, and interdisciplinary teams

Monitor patient progress and adjust care plans as necessary

Educate patients and families on medications, treatments, and disease management

Complete accurate and timely OASIS documentation

Ensure compliance with Medicare, Medicaid, and agency policies

Promote patient safety, independence, and improved quality of life

Requirements:

Active North Carolina RN license

Minimum 1–2 years of recent home health and/or case management experience

Current BLS certification

OASIS documentation experience preferred

Strong communication, assessment, and organizational skills

Valid driver’s license and reliable transportation required
