Seven Healthcare is proud to present a great opportunity for a Travel Home Health RN in Clifton, New Jersey. This assignment includes a weekly blended rate of $2,200, based on five 8‑hour day shifts. This role is ideal for an RN who enjoys independent practice, strong patient interaction, and supporting clients in their homes.

Key Responsibilities of the Home Health RN

Conduct in‑home assessments to determine clinical needs and home‑environment safety
Develop individualized care plans and collaborate with healthcare providers
Provide wound care, injections, medication management, and IV therapy
Educate patients and families on chronic‑condition management and safe care techniques
Maintain accurate, timely visit documentation in EMR
Coordinate services with therapy teams, case managers, and physicians
Manage caseload travel, scheduling, and daily patient prioritization
Support safe transitions and ongoing continuity of care

Requirements of the Home Health RN

Active New Jersey RN license
Current BLS certification
Minimum 1 year of recent Home Health RN experience
Reliable transportation for community visits
