Home Health RN Job Details

• Position: Home Health Registered Nurse (RN)
• Location: Neenah, WI (serving Appleton and nearby Wisconsin communities)
• Employment Type: Travel Nursing Contract
• Start Date: October 20, 2025
• Duration: 13 weeks
• Shifts per Week: 5 (Monday - Friday)
• Shift Duration: 8-hour day shifts (8:00 AM - 5:00 PM)
• Hours per Week: 40
• Estimated Weekly Pay: $1,755 - $1,950
• No weekends or on-call required

Registered Nurse Qualifications

• Active Wisconsin or Compact RN License – required
• Minimum 12 months of recent Home Health RN experience within the last 3 years
• Proficiency with HCHB (Homecare Homebase) – required
• Experience with OASIS documentation – required
• BLS certification – required (must be current at time of application)

Home Health RN Responsibilities

• Provide skilled nursing care to patients in their homes throughout Neenah, Appleton, and the Fox River Valley
• Conduct comprehensive patient assessments and develop individualized care plans
• Document patient care accurately using HCHB and OASIS systems
• Collaborate with interdisciplinary healthcare teams to achieve optimal patient outcomes
• Meet productivity expectations of 30 points per week
• Deliver compassionate, patient-centered care while maintaining high standards of clinical excellence
