Home Health RN Job Details
- Location: Medford, OR and surrounding areas (97504)
- Assignment Type: Travel Nursing

- Nursing Shift: 5x8-hour day shifts (08:00 - 16:30)
- Hours per Week: 40
- Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)
- On-Call: Not required
- Weekend Requirement: No weekends required
- Charting System: EPIC/OASIS
- Scrub Color: Any color
- Parking: Free
- Mileage Reimbursement: IRS Standard
Registered Nurse Qualifications
- Active Registered Nurse (RN) license in Oregon (pending license accepted)
- Minimum 1 year of recent skilled adult home health nursing experience
- BCLS certification required
- OASIS documentation experience required
- Reliable transportation required (occasional travel up to 50+ miles; mileage reimbursed)
- COVID-19 primary series vaccination and one booster required
- Ability to provide two supervisory references covering at least one year within the last three years in a comparable specialty/setting
- Strong clinical skills in admission assessment, wound care, medication administration/management, infection control, and computerized documentation
Homecare RN Responsibilities
- Deliver skilled nursing care to adult patients in their homes across Medford and surrounding areas
- Conduct admission assessments, routine visits, recertifications, resumptions of care, and discharges
- Provide wound care, pain assessment and management, and medication administration (IV, IM, subcutaneous, intradermal)
- Utilize OASIS for accurate and timely documentation
- Maintain infection control precautions and follow best practices for patient safety
- Coordinate care with interdisciplinary healthcare teams and case management
- Manage a daily caseload of up to 6 patients, meeting productivity standards
- Travel efficiently between patient homes with support to minimize drive time
