Registered Nurse Job Details

• Schedule: Monday – Friday, 8:00 AM – 5:00 PM

• Shift Duration: 8-hour day shifts

• EMR: HCHB (Homecare Homebase)

RN Qualifications

• Active Washington State Registered Nurse (RN) license
• At least 1 year of home health nursing experience in a Medicare-certified healthcare facility
• Proficient with HCHB (Homecare Homebase) EMR system
• Ability to work independently and travel to patient homes across King County
• Strong clinical assessment, communication, and organizational skills

Home Health Nurse Responsibilities

• Deliver skilled nursing care to patients in their homes throughout West Seattle and King County
• Create, implement, and evaluate individualized patient care plans
• Collaborate with interdisciplinary healthcare teams for comprehensive patient support
• Accurately document patient care using HCHB EMR
• Educate patients and families on disease management, medications, and self-care
• Maintain productivity standards (30 points for 40 hours)
