Home Health RN Job Details
- Location: Norfolk, VA 23502 and surrounding areas (travel radius up to 75 miles)
- Assignment Length: 13-week travel contract
- Start Date: January 12, 2026
- Schedule: Monday – Friday, 8:00 AM – 5:00 PM (5x8-hour day shifts)
- Hours per Week: 40 (based on productivity points; no guaranteed hours)
- Travel: Home visits required; periodic office meetings
- Mileage: Reimbursed at IRS standard rate (patient home to patient home)
Registered Nurse Qualifications
- Active Registered Nurse (RN) license in Virginia (must have license in hand)
- Minimum 1 year of recent Adult Home Health or Hospice RN experience
- Current CPR/BLS certification
- Proficiency in HomeCare HomeBase (HCHB) EMR (recent experience or willingness to retrain)
- Proficiency in OASIS documentation (recent experience or willingness to retrain)
- Valid driver's license, reliable transportation, and auto insurance
- Willingness to travel up to 75 miles from the home clinic
- Ability to complete all documentation within 24 hours of patient visit
- Open to on-call and weekend rotations as needed
- Not a current direct employee of the agency if applying as a traveler
Home Health RN Responsibilities
- Provide skilled nursing care to patients in their homes following physician orders and agency policies
- Supervise and evaluate Home Health Aides and LPNs/LVNs
- Conduct patient assessments, develop care plans, and deliver wound care, start-of-care visits, and follow-up visits
- Manage a daily caseload of 5–8 patients (6–8 productivity points per day; 30 points per week)
- Travel between patient homes and attend office meetings as required
- Ensure timely and accurate documentation using HCHB and OASIS
- Participate in on-call and weekend rotations
- Maintain compliance with all agency and healthcare regulatory requirements
