Connecticut Nursing Job Details

• Location: Bristol, CT 06010 – Connecticut hospice nursing jobs
• Setting: Home Health and Hospice
• Employment Type: Travel RN Contract
• Start Date: 12/26/2025
• End Date: 3/27/2026
• Duration: 13-week nursing contract
• Schedule: Monday – Friday, 8:00am – 4:30pm (8-hour shifts)
• Guaranteed Hours: 40 hours per week
• Weekend Call: Every 4th weekend (flexible weekday off)
• Self-Scheduling: Yes
• Orientation: 1 week with preceptor
• Dress Code: Any color scrubs, clean and professional

Registered Nurse Qualifications

• Active Connecticut RN license (must be licensed and fingerprinted in CT)
• BLS Certification (Basic Life Support)
• Valid driver's license and auto insurance
• Reliable vehicle for travel within a 20-minute radius
• Minimum 1 year of home care or hospice nursing experience required
• Oasis EMR experience highly preferred
• Experience with adult and geriatric patient populations

RN Hospice Job Responsibilities

• Deliver compassionate home-based hospice care to adult and geriatric patients
• Perform patient assessments, develop and implement individualized care plans, and manage medications
• Care for patients with diagnoses such as CHF, COPD, post-surgical, orthopedic, CVA, dementia, and hospice needs
• Collaborate with Home Health/Hospice Aides and the interdisciplinary healthcare team
• Document patient care using Oasis (Home Health) charting system
• Complete an average of 5-6 patient visits per day, utilizing a point system for visit types
• Participate in weekend call rotation, responding to calls within 20 minutes and visiting within 30 minutes as needed
• Engage in utilization review and quality improvement activities
