Registered Nurse Qualifications

Active Registered Nurse (RN) license in Washington State
At least 1 year of recent home health nursing experience with a Medicare Certified Agency
Experience with Home and Community-Based Services (HCHBS) preferred
Ability to travel within Lewis County and nearby areas as needed
Excellent communication, organizational, and assessment skills

Home Health RN Responsibilities

Deliver skilled nursing care to patients in their homes, ensuring high-quality and compassionate clinical support
Conduct comprehensive patient assessments, develop individualized care plans, and monitor patient progress
Educate patients and families on health management, chronic disease care, and self-care techniques
Collaborate with interdisciplinary healthcare team members to provide coordinated, patient-centered care
Accurately document all nursing care activities in accordance with home health agency policies
Adapt to changing patient needs and travel within the designated territory in Lacey and Lewis County
