• Duration: 13 weeks
• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• Shift Duration: 8-hour days
• Hours Per Week: 40 guaranteed
• On-call/Weekend Requirements: None
• Territory: Primarily West Seattle; may see patients throughout King County
• Estimated Weekly Pay: $2350 - $2470
• EMR: HCHB (Homecare Homebase) required
• Productivity: 30 points per 40 hours/week

Job Requirements

• Minimum 1 year of home health RN experience in a Medicare-certified facility (home health nurse experience)
• Active Registered Nurse (RN) license eligible for practice in Washington State (compact license holders must verify WA eligibility)
• Experience with HCHB (Homecare Homebase) EMR required
• Strong assessment, care planning, and patient education skills for home care nursing
• Ability to work independently, efficiently manage a daily route, and prioritize patient visits
• Reliable transportation for travel within the assigned territory (King County travel required)

Responsibilities

• Deliver skilled nursing care to patients in their homes according to physician orders and individualized care plans
• Conduct comprehensive home health patient assessments, monitor changes in condition, and communicate updates to the interdisciplinary care team
• Educate patients and caregivers on medications, chronic disease management, safety, and symptom monitoring
• Coordinate care with social workers, therapists, physicians, and other team members to ensure continuity and quality outcomes
• Document all visits accurately and promptly in HCHB, meeting Medicare and state regulatory standards
• Maintain productivity expectations while providing compassionate, patient-centered home care
