Home Health RN Qualifications

• Minimum 2 years of experience as a Home Health Registered Nurse (RN)
• Proficiency with OASIS and HCHB documentation systems
• Active RN license in the state of Kentucky
• Current CPR certification
• Valid driver's license, vehicle insurance, and reliable transportation

Home Health Nursing Responsibilities

• Provide skilled nursing care to patients in their homes according to individualized care plans
• Conduct comprehensive patient assessments and document care using OASIS and HCHB systems
• Collaborate with interdisciplinary healthcare teams to achieve optimal patient outcomes
• Educate patients and families on disease management, medication administration, and self-care techniques
• Maintain accurate and timely documentation of all patient care activities
