Position: Licensed Practical Nurse (LPN) – Long-Term Care (LTC), Skilled Nursing
Location: Rutland, Vermont (Rutland, VT 05701) — Vermont nursing jobs, Rutland healthcare
Assignment Type: Travel Contract (Local Only) — local travel nurse / LPN travel contract
Start Date: February 2, 2026
End Date: April 27, 2026
Duration: 12 weeks
Shift: 8-hour Nights (10:00 PM – 6:30 AM) — Night Shift LPN / LTC nurse
Hours per Week: 40
Estimated Weekly Pay: $1,250 – $1,310
Weekend Commitment: Every weekend required
Transportation: Must have own transportation

Job Requirements / Qualifications

Active LPN license in Vermont or eligibility via multistate compact (Vermont LPN license preferred)
Experience with medication administration, resident assessments, documentation (EMR/charting), and ADL support
Ability to work night shifts and every weekend as scheduled (Night Shift LPN, LTC nurse)
Strong communication, teamwork, and resident-centered care skills
Reliable transportation required

Responsibilities

Provide compassionate, high-quality nursing care to residents in a long-term care/skilled nursing facility
Administer medications and treatments as prescribed and document accurately in the EMR
Perform routine assessments, monitor residents’ health status, and report changes to the interdisciplinary team
Assist residents with activities of daily living (ADLs) and promote dignity and independence
Collaborate with nursing staff, nursing assistants, physicians, therapists, and other healthcare professionals for comprehensive care planning
Maintain accurate clinical records, follow facility policies, and adhere to infection control and safety protocols
Support a positive resident experience and contribute to quality improvement initiatives
