Travel Home Health RN Requirements

• Experience: Minimum 2 years of Home Health RN experience (required for submission)
• License: Active California RN license (required at time of submission)
• Certifications: BLS (AHA or American Red Cross) in hand at time of submission
• Documentation: OASIS experience and strong clinical documentation skills
• Charting: Charting system experience required; Epic used (Epic preferred)
• Patient Population: Adults and geriatrics
• Compliance Readiness: Ability to complete lab work within 3–5 days of signing
• References: 2 professional references from within the last year (required)
• Work History: Ability to provide 7-year work history for verification
• Travel Requirement: Must meet a 50-mile travel requirement

Home Health Registered Nurse (RN) Responsibilities

• Deliver skilled nursing care in the home in alignment with physician orders, professional standards, and agency protocols
• Complete home health visit types including Start of Care (SOC), Resumption of Care (ROC), recertifications, routine visits, transfers, and significant change in condition assessments
• Implement and evaluate individualized plans of care; obtain and communicate physician orders as needed
• Coordinate care within an interdisciplinary team and support effective utilization management and appropriate supply use
• Educate patients and caregivers on medications, disease management, safety, and care plans tailored to learning needs and the home environment
• Perform comprehensive assessments and medication reconciliation; support pain assessment and management
• Provide wound/skin care (including pressure injury prevention/staging) and wound VAC management as applicable
• Support home-based therapies and devices as applicable (e.g., IV therapy/infusions, line care, oxygen safety/administration, tracheostomy care, enteral feeding tubes, ostomy care)
• Document accurately and on time in Epic, including required OASIS documentation
• Supervise home health aides as assigned and promote safe, effective care delivery
