• Location: Portage, MI 49002 (serving St Joseph & Branch Counties; near Kalamazoo)
• Setting: Home Health (community/home visits)
• Employment Type: Travel Contract – PTA travel job
• Contract Duration: 13 weeks (March 30, 2026 – June 28, 2026)
• Schedule: Monday – Friday
• Shift Duration: 8-hour days
• Hours per Week: 40
• Estimated Weekly Pay Range: $1,400 – $1,500

Job Requirements

• Active Physical Therapist Assistant (PTA) certification/licensure (Michigan license preferred or eligible)
• Home health experience required
• Ability to work independently in home health settings and collaboratively within a multidisciplinary team
• Strong communication, documentation (EMR/charting), and organizational skills
• Reliable transportation to travel between patient locations across St Joseph & Branch Counties

Responsibilities

• Assist Physical Therapists in delivering high-quality rehabilitative care to home health patients, including home safety assessments and discharge planning as appropriate
• Implement and follow individualized treatment plans, including therapeutic exercise, gait training, transfers, and ADL retraining; monitor patient progress and report changes to the supervising PT
• Accurately document patient care activities, functional outcomes, progress notes, and discharge plans in the EMR
• Provide patient and family education on therapy goals, safety, home exercise programs, and fall prevention
• Collaborate with nurses, physicians, case managers, and the interdisciplinary team to optimize patient outcomes and transitions of care
• Travel to patient residences within the assigned Portage/Kalamazoo/St Joseph/Branch County territory to provide in-home therapy services as needed
