  • End Date: June 29, 2026
  • Shift: 8 Hour Days (08:00 – 17:00), Monday–Friday (no weekends, no holidays)

Job Requirements (Qualifications)

  • Experience: Minimum 3 years of RN experience required (first-time travelers accepted)
  • License: DC RN license in hand at time of submission (Washington DC nursing license)
  • Certifications: BLS (AHA)
  • Preferred: Ambulatory/clinic and cardiology or heart failure nursing experience; prior nurse navigator or case management experience is a plus
  • Charting: Cerner EMR experience required (ambulatory documentation, medication reconciliation)
  • References: Two supervisor references required
  • Vaccines: Flu vaccine required (no exemptions)

Responsibilities (Heart Failure Nurse Navigator / Ambulatory Care)

  • Serve as Advanced Heart Failure Nurse Navigator: coordinate care, care management, and navigation for patients with complex heart failure in an ambulatory/clinic setting
  • Perform comprehensive patient assessments, medication administration, and medication reconciliation (cardiac medication management)
  • Interpret laboratory and imaging results and communicate findings to cardiology providers
  • Deliver patient education focused on heart failure self-management, symptom recognition, lifestyle modification, and medication adherence
  • Provide telephone triage, follow-up calls, telephonic care coordination, and transitions-of-care planning to ensure continuity and reduce readmissions
  • Prepare charts, complete disability/authorization forms, coordinate referrals, and manage medication refills
  • Document accurately and efficiently in Cerner EMR (ambulatory documentation best practices)
  • Collaborate with interdisciplinary teams including interpretation services, phlebotomy, social work, pharmacy, and rapid response teams
  • Maintain universal precautions and adhere to infection control protocols
