Job Details
- Position: Licensed Practical Nurse (LPN) – Home Health / Long-Term Care (LTC)
- Job Type: Travel Contract (Travel LPN, 13 weeks)
- Location: Wichita, KS 67202 — serving a 50-mile radius across the Metro Wichita 5-county area (South Central Kansas)
- Start / End Dates: March 23, 2026 – June 21, 2026
- Shift: Days, Monday–Friday, 8-hour shifts (40-hour weekly guarantee)
- Estimated Weekly Pay: $1,420 – $1,500
Qualifications / Job Requirements
- Active Licensed Practical Nurse (LPN) license in Kansas
- Home Health nursing experience required
- Proficiency with Homecare Homebase (HCHB) and OASIS documentation required
- Valid driver’s license and reliable transportation to travel throughout the assigned territory
- Strong communication, clinical assessment, patient education, and care coordination skills
Key Responsibilities
- Provide compassionate, high-quality in-home nursing care to patients across Metro Wichita (home health nursing and long-term care)
- Perform patient assessments, develop and update individualized care plans, and deliver ongoing monitoring and skilled interventions
- Document all care, visits, and patient interactions accurately using HCHB and OASIS systems
- Coordinate care with interdisciplinary teams including RNs, therapists, social workers, and physicians
- Educate patients and families on disease management, medication administration, wound care, and home safety
- Maintain compliance with agency policies, state regulations, and home health clinical standards
