Job Details
- Position: Travel Home Health Registered Nurse (RN) — Travel Nurse / Home Health RN
- Location: Lacey, WA 98503 (field-based home health; coverage across Thurston, Lewis, Mason, and Pierce counties)
- Assignment Length: 13 weeks (March 23, 2026 – June 21, 2026)
- Schedule: Monday–Friday, 8:00 AM – 5:00 PM
- Shift Duration: 8-hour days
- Guaranteed Hours: 40 hours per week
- Estimated Weekly Pay: $2,420 – $2,550 (competitive pay for travel nurse & home health RN)
- Territory: Home health visits across Thurston, Lewis, Mason, and Pierce counties
- On-call/Weekend Requirements: To be determined
- EMR: Homecare Homebase (HCHB) required — HCHB experience preferred
Job Requirements / Qualifications
- Active Registered Nurse (RN) license (state licensing per assignment requirements)
- Minimum 1 year of home health RN experience with a Medicare-certified facility (home health nursing experience)
- Proficiency with Homecare Homebase (HCHB) electronic medical records
- Ability to travel throughout the assigned coverage area for home health visits
Responsibilities
- Deliver skilled nursing care and field-based home health nursing to patients in their homes, ensuring high-quality, compassionate service
- Complete timely and accurate documentation in Homecare Homebase (HCHB) for each home health visit
- Meet productivity expectations of 6 points per day and 30 points per week for home health assignments
- Collaborate with interdisciplinary teams (PT, OT, social work, physicians) to develop and implement individualized patient care plans
- Provide patient and family education on care plans, medications, wound care, disease management, and discharge planning
