- End Date: June 29, 2026
- Shift: 8:00 AM – 4:30 PM (8-hour days)
- Hours: 40 hours/week guaranteed
- On-Call: None
- Weekend/Holiday: Every 4–6 weekends; holiday coverage as needed
- Estimated Weekly Pay: $2,260 – $2,430

Job Requirements / Qualifications
- Minimum 2 years of home health Speech-Language Pathologist (SLP) experience (required)
- Master’s or Doctorate degree in Speech-Language Pathology (required)
- Certificate of Clinical Competence (CCC-SLP) required; Clinical Fellow (CF) candidates accepted
- Active Massachusetts Speech-Language Pathologist license (required)
- Active American Heart Association BLS certification (required)
- Valid driver’s license and reliable personal vehicle for home visits (required)
- Ability to work independently in the community with minimal supervision (home health independence)
- Familiarity with rehabilitation equipment, AAC devices, communication aids, and mobility equipment
- Acute care experience preferred (beneficial for complex dysphagia cases)
- Work history, up-to-date skills checklist (within 1 year), and one supervisory reference (within past 2 years) required for profile submission
- Must not reside within 50 miles of the facility (travel assignment requirement)
Key Responsibilities
- Provide direct speech-language pathology services in patients' homes, including evaluation, individualized treatment, reassessment, and discharge planning
- Perform comprehensive dysphagia assessment and management, including swallowing safety evaluations and implementation of compensatory and rehabilitative strategies
- Train patients and caregivers on communication strategies, augmentative and alternative communication (AAC) tools, home exercise programs, and safe swallowing techniques
- Collaborate with interdisciplinary home health team members (nurses, PT/OT, social workers) to develop and implement comprehensive care plans
- Document evaluations, progress notes, care coordination, and outcomes in the agency EMR in a timely and accurate manner
- Utilize rehabilitation and assistive communication equipment to support patient goals and functional outcomes
- Participate in agency meetings, case reviews, and quality improvement initiatives to support best-practice home health services
