• Unit: CVMC WDR - Maple Grove (Long Term Care / Rehabilitation)
• Assignment Type: Travel Contract (Travel LPN)
• Start Date: March 9, 2026
• End Date: April 20, 2026
• Duration: 6 weeks
• Shift: 12-hour nights (Night Shift LPN)
• Hours per Week: 36
• Schedule: Every other weekend required
• Estimated Weekly Salary: $1,680–$1,770 per week

Job Requirements (LPN / Travel Nurse Qualifications)

• Active Licensed Practical Nurse (LPN) license in the U.S.
• BLS certification through AHA (American Heart Association) required
• Minimum 3 years of LPN experience, including Long Term Care (LTC) experience
• Prior travel nursing / travel LPN experience required
• Comfortable and experienced working with dementia and memory care residents
• Willingness to float to other units as needed to support facility operations
• Ability to work assigned night shifts and every other weekend (no block scheduling)
• Reliable personal or rental vehicle required (public transportation not accessible)
• Vehicles should have all-season or snow tires November–April
• All employment gaps over 90 days must be explained
• Complete work history required for submission (include bed count, EMR, facility type, unit)
• 7-year background check (nationwide, county court, FACIS3, sex offender) within 18 months of start date
• FACIS 3 and VCIC checks within 30 days of start

Responsibilities (Long Term Care LPN)

• Provide compassionate, high-quality nursing care to residents in a long term care and rehabilitation setting
• Administer medications and treatments as ordered and document per facility policy
• Monitor, assess, and document resident conditions; promptly report changes to the interdisciplinary healthcare team
• Collaborate with nurses, CNAs, therapists, social work, and physicians to ensure comprehensive resident care
• Support residents with dementia, behavioral changes, and complex care needs using person-centered approaches
• Float to other units as needed to maintain staffing and continuity of care
• Maintain compliance with facility policies, state regulations, and long term care best practices
• Communicate effectively with residents, families, and staff to promote resident-centered care
