• Duration: 7 weeks (travel assignment / travel nurse)
• Shift: 12-hour day shifts

Job Requirements

• Active Registered Nurse (RN) license in Wisconsin or valid compact state license
• Minimum 1 year of nursing experience required; hospice, homecare, or palliative care experience preferred
• Current Basic Life Support (BLS) certification (AHA preferred)
• Strong clinical assessment, symptom management, and patient education skills
• Excellent communication, documentation, and electronic medical record (EMR) abilities
• Ability to work independently during home visits and collaborate with an interdisciplinary hospice team
• Valid driver’s license and reliable transportation for home healthcare visits

Key Responsibilities

• Provide compassionate, patient-centered hospice care in patients’ homes as a Hospice RN / Home Health RN
• Assess patient conditions, manage complex symptoms, and develop individualized care plans
• Administer medications and treatments per physician orders and hospice protocols
• Educate patients and families on disease progression, comfort measures, and end-of-life support
• Coordinate care with physicians, social workers, chaplains, and other healthcare professionals
• Accurately document patient encounters, care plans, and progress notes in a timely manner
• Offer emotional support and guidance to patients and families during end-of-life care
