Job Requirements / Qualifications
- Active Registered Nurse (RN) license in Washington State (required)
- Minimum 1 year of home health nursing experience with a Medicare-certified facility
- Proficiency with HCHB (Homecare Homebase) EMR
- Valid driver’s license, reliable transportation, and ability to travel within assigned counties
- Strong communication, documentation, and organizational skills for home health visits and care coordination
Key Responsibilities (Home Health / Skilled Nursing)
- Provide skilled nursing care to patients in their homes across Thurston, Lewis, Mason, and Pierce counties
- Manage and maintain an assigned caseload with productivity expectations (6 points/day; 30 points/week)
- Develop, implement, and evaluate individualized patient care plans in collaboration with the interdisciplinary home health team
- Perform skilled nursing tasks and clinical assessments and document accurately in HCHB EMR
- Educate patients and families on disease processes, medication management, wound care and self-care techniques to promote independent living
- Coordinate with case managers, therapists, and primary care clinicians to ensure continuity of care and optimal patient outcomes
