Job Requirements / Qualifications

• Active Registered Nurse (RN) license in Washington State (required)
• Minimum 1 year of home health nursing experience with a Medicare-certified facility
• Proficiency with HCHB (Homecare Homebase) EMR
• Valid driver’s license, reliable transportation, and ability to travel within assigned counties
• Strong communication, documentation, and organizational skills for home health visits and care coordination

Key Responsibilities (Home Health / Skilled Nursing)

• Provide skilled nursing care to patients in their homes across Thurston, Lewis, Mason, and Pierce counties
• Manage and maintain an assigned caseload with productivity expectations (6 points/day; 30 points/week)
• Develop, implement, and evaluate individualized patient care plans in collaboration with the interdisciplinary home health team
• Perform skilled nursing tasks and clinical assessments and document accurately in HCHB EMR
• Educate patients and families on disease processes, medication management, wound care and self-care techniques to promote independent living
• Coordinate with case managers, therapists, and primary care clinicians to ensure continuity of care and optimal patient outcomes
