• Assignment Type: Travel — Homecare RN / Travel Nurse

• Duration: 13 weeks
• Shift: 12-hour day shifts (Friday–Sunday or Saturday–Monday)
• Hours per Week: 36
• Territory: Greater Milwaukee area — travel nursing coverage across Milwaukee neighborhoods
• Estimated Weekly Pay: $1730–$1820

Job Requirements

• Active Registered Nurse (RN) license in the state of Wisconsin (required for home health RN / travel nurse)
• Minimum 1 year of Home Health nursing experience within the past 3 years
• Proficient with HCHB (Homecare Homebase) and OASIS documentation
• Strong clinical assessment, organizational, and communication skills for home health settings
• Valid driver’s license and reliable transportation for travel nursing visits across Milwaukee

Responsibilities

• Provide skilled nursing care in patients' homes across the Greater Milwaukee area (home health RN duties)
• Complete three admissions per day plus approximately two additional skilled visits per day
• Document accurately and timely using HCHB and OASIS to support clinical records and reimbursement
• Collaborate with interdisciplinary home health team members to develop and implement individualized care plans
• Educate patients and families on disease processes, medication management, wound care, and self-care techniques
• Deliver high-quality, compassionate care that promotes patient independence, safety, and well-being
