- Shift: Day shifts, 5x8-hour (07:00 - 15:00), Monday–Friday

- Productivity: 30 points/week (~40 hours/week)
- Average Patients per Day: 5–8
- Mileage: Travel up to 75 miles from home clinic; mileage reimbursed at IRS standard rate (from patient home to patient home)
- Weekend/On-Call: 1 weekend and 1 on-call week per month (varies by location)
- Estimated Weekly Salary: $2390–$2520/week
Job Requirements (Home Health RN Qualifications)
- Active Registered Nurse (RN) license in Oregon (license in hand required)
- Minimum 1 year of recent adult Home Health or Hospice experience (preferred experience for Home Health RN positions)
- Current CPR/BLS certification
- Proficiency with HomeCare HomeBase (HCHB) EMR and OASIS (used within last 6 months or willing to retrain)
- Valid driver's license, reliable vehicle, and current auto insurance
- Ability to travel up to 75 miles from assigned care center between patient homes
- Willingness to participate in on-call and weekend rotations
- Strong clinical assessment, documentation, and time-management skills (charting in HCHB within 24 hours)
- Not currently a direct employee of the facility if applying as a traveler
Key Responsibilities (Home Health RN / Travel RN Duties)
- Provide skilled nursing care in patients' homes per physician orders and agency policies (home health nursing, RN home care)
- Perform comprehensive assessments, admissions, recertifications, and routine visits using OASIS and HCHB EMR
- Deliver wound care, medication management, pain assessment, and patient/family education
- Manage complex cases including G-tube and NG tube care, tracheostomy and ventilator care, and IV therapy
- Supervise and evaluate Home Health Aides and LPNs/LVNs when applicable
- Document all patient care activities accurately and timely in HCHB (within 24 hours of visit)
- Participate in interdisciplinary team meetings and periodic office meetings
- Maintain productivity expectations while traveling between patient homes
- Adhere to infection control, safety protocols, and agency compliance standards
