Job Requirements / Qualifications
- Minimum 2 years of recent hospital pharmacy or inpatient pharmacy experience required
- Current State Pharmacist Licensure (South Carolina) required for travel pharmacist roles
- Successful completion of educational requirements for state licensure
- EPIC experience required (electronic medical record / pharmacy informatics experience)
- BLS certification (if required by facility/state)
- Strong communication and interdisciplinary collaboration skills for patient/family education and clinical rounding
Primary Responsibilities
- Provide inpatient medication dispensing and clinical pharmacy services, including STAT and emergency order processing
- Maintain adequate drug supply, manage procurement, and ensure inventory control for the hospital pharmacy
- Prepare, compound, sterilize, and label parenteral and IV medications in compliance with sterile compounding standards
- Oversee controlled substance accountability and documentation per state and federal regulations
- Perform medication reconciliation and prepare discharge medication lists; provide discharge counseling and medication education to patients and families
- Monitor drug therapy regimens for safety and effectiveness, including antimicrobial stewardship, pharmacokinetic dosing, and anticoagulation management
- Participate in quality assurance, medication safety initiatives, and Pharmacy & Therapeutics Committee meetings
- Support, train, and mentor pharmacy staff and collaborate with nursing and medical teams to optimize patient outcomes
- Conduct monthly inspections of pharmacy and patient care areas and ensure compliance with hospital policies and safety standards
