Job Details – Travel RN / Home Health Nurse

• Location: Saint Albans, Vermont (05478) — Vermont nursing jobs / Saint Albans healthcare
• Assignment Type: Travel (Travel Nursing / Home Health RN)
• Duration: 13 weeks (March 30, 2026 – June 29, 2026)
• Hours: 40 hours per week (Guaranteed 37 hours)
• Shift: Tuesday – Saturday, 8:00 AM – 4:00 PM (8-hour days)
• On-call rotation: Required (Includes hospice coverage when on-call)
• Estimated Weekly Salary: $2,500 - $2,630 per week

Required Qualifications – Homecare Registered Nurse

• Active Registered Nurse (RN) license in Vermont (compact license not accepted unless valid in VT)
• Minimum 1 year recent Home Health RN or Hospice RN experience
• Certification in Nursing (relevant RN certifications)
• Strong knowledge and proficiency with OASIS documentation and federal/state Home Health requirements
• Clinical skills: IV use and maintenance, wound VAC, complex wound care, high-tech home nursing skills
• Ability to participate in on-call rotation and provide hospice coverage as needed
• Excellent communication, organizational, and patient education skills

Key Responsibilities – Home Health / Hospice RN

• Perform comprehensive nursing assessments for homecare patients using the nursing process
• Develop, document, and communicate individualized plans of care incorporating patient and caregiver goals
• Serve as Case Manager for assigned patients; manage caseloads, schedule visits, and coordinate care
• Complete accurate and timely OASIS documentation and required agency, state, and federal forms
• Implement medical plans of care, nursing interventions, and advanced procedures in the home setting
• Maintain infection control and safety standards across all patient visits
• Collaborate with LPNs, PTs, social workers, and other interdisciplinary team members; delegate tasks appropriately
• Evaluate patient responses, update care plans, and advocate for patient-centered outcomes
• Provide patient and caregiver education on medications, treatments, wound care, and discharge instructions
• Participate in monthly peer review audits, case conferences, and quality improvement activities
