Qualifications / Job Requirements
- Active LPN/LVN license in the required state (LPN license)
- BLS Certification (Basic Life Support) from AHA or equivalent
- Minimum 1 year of experience in long-term care, post-acute care, or similar setting
- Comfortable and reliable working 12-hour night shifts on a blocked schedule
- Strong communication, teamwork, and resident-centered care skills
- Experience with electronic charting (TruBridge - Evident) preferred
Key Responsibilities — LPN Long-Term Care
- Provide compassionate, resident-centered nursing care in a long-term care facility
- Administer medications and treatments accurately and chart using TruBridge - Evident
- Monitor and document residents’ vital signs, conditions, and progress
- Collaborate with interdisciplinary team members (RNs, CNAs, therapists, social workers) to coordinate care
- Assist and support residents with activities of daily living while promoting dignity and independence
- Respond promptly to changes in residents’ conditions and communicate with families and clinical staff
Skills & Competencies
- Clinical assessment and medication administration
- Strong time management and clinical prioritization on night shifts
- Interpersonal skills for working with residents, families, and staff
- Familiarity with long-term care regulations and best practices
