Qualifications / Job Requirements

• Active LPN/LVN license in the required state (LPN license)
• BLS Certification (Basic Life Support) from AHA or equivalent
• Minimum 1 year of experience in long-term care, post-acute care, or similar setting
• Comfortable and reliable working 12-hour night shifts on a blocked schedule
• Strong communication, teamwork, and resident-centered care skills
• Experience with electronic charting (TruBridge - Evident) preferred

Key Responsibilities — LPN Long-Term Care

• Provide compassionate, resident-centered nursing care in a long-term care facility
• Administer medications and treatments accurately and chart using TruBridge - Evident
• Monitor and document residents’ vital signs, conditions, and progress
• Collaborate with interdisciplinary team members (RNs, CNAs, therapists, social workers) to coordinate care
• Assist and support residents with activities of daily living while promoting dignity and independence
• Respond promptly to changes in residents’ conditions and communicate with families and clinical staff

Skills & Competencies

• Clinical assessment and medication administration
• Strong time management and clinical prioritization on night shifts
• Interpersonal skills for working with residents, families, and staff
• Familiarity with long-term care regulations and best practices
