Job Requirements

• Active Registered Nurse (RN) license (state required prior to start)
• BLS Certification (required prior to first shift)
• ACLS Certification (required prior to first shift)
• Minimum 2 years of bedside clinical experience; LTAC, sub-acute, or similar acute/rehabilitation experience preferred
• Experience with complex patients, ventilator care, wound management, and interdisciplinary rehabilitation teams is a plus

Responsibilities

• Perform comprehensive nursing assessments on all patients within 8 hours of admission
• Develop individualized care plans in collaboration with patients, families, and interdisciplinary teams
• Provide direct patient care following the nursing process: assessment, planning, implementation, and evaluation
• Document assessments, interventions, and patient responses accurately and in a timely manner
• Initiate discharge planning and patient teaching beginning at admission
• Communicate changes in patient status to care providers to ensure continuity and patient safety
• Supervise and provide direction to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees
• Promote patient and staff safety by adhering to facility guidelines and safety protocols
• Participate in team building and contribute to a positive work environment
• Assume charge nurse responsibilities as needed
• Engage in ongoing professional development and continuing education
