Job Requirements
- Active Registered Nurse (RN) license (state required prior to start)
- BLS Certification (required prior to first shift)
- ACLS Certification (required prior to first shift)
- Minimum 2 years of bedside clinical experience; LTAC, sub-acute, or similar acute/rehabilitation experience preferred
- Experience with complex patients, ventilator care, wound management, and interdisciplinary rehabilitation teams is a plus
Responsibilities
- Perform comprehensive nursing assessments on all patients within 8 hours of admission
- Develop individualized care plans in collaboration with patients, families, and interdisciplinary teams
- Provide direct patient care following the nursing process: assessment, planning, implementation, and evaluation
- Document assessments, interventions, and patient responses accurately and in a timely manner
- Initiate discharge planning and patient teaching beginning at admission
- Communicate changes in patient status to care providers to ensure continuity and patient safety
- Supervise and provide direction to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees
- Promote patient and staff safety by adhering to facility guidelines and safety protocols
- Participate in team building and contribute to a positive work environment
- Assume charge nurse responsibilities as needed
- Engage in ongoing professional development and continuing education
