Job Requirements

• Active Registered Nurse (RN) license
• BLS Certification (required prior to first shift)
• ACLS Certification (required prior to first shift)
• Minimum 1 year of bedside clinical experience (2 years highly desired)
• Experience in LTAC, long-term acute care, step-down, or similar acute care settings preferred

Key Responsibilities

• Perform comprehensive nursing assessments within 8 hours of patient admission
• Develop individualized care plans with patients, families, and interdisciplinary teams
• Provide direct patient care using the nursing process: assessment, planning, implementation, and evaluation
• Accurately document assessments, interventions, and patient responses in the electronic health record
• Initiate and coordinate patient teaching and discharge planning starting at admission
• Communicate clinical changes to providers to ensure continuity and patient safety
• Supervise and provide direction to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees
• Promote patient and staff safety by adhering to facility policies, infection control, and regulatory standards
• Participate in team building and positively contribute to unit operations; assume charge nurse duties as needed
• Engage in ongoing professional development and continuing education relevant to LTAC and critical care nursing

Clinical Skills & Experience (Preferred)

• Experience with LTAC patient populations, including post-acute care and rehabilitation
• Familiarity with ventilator support, tracheostomy care, wound care, and complex medication regimens (may be required depending on unit scope)
• Strong assessment, critical thinking, and care coordination skills
