Location: Chesapeake, VA (23320) — Home Health / Skilled Nursing
Assignment Type: Travel RN / Travel Nursing
Setting: Home Health, Homecare, Skilled Nursing
Duration: 13 weeks (April 20, 2026 – July 19, 2026)
Hours: 40 hours per week
Shift: 5 × 8-hour shifts per week (Any shift)

Compensation

Estimated Weekly Pay: $1,700 - $1,790 per week

Key Qualifications / Job Requirements

Active Registered Nurse (RN) license in Virginia
Minimum 1 year recent experience in Home Health or Skilled Nursing (Homecare RN, Home Health Nurse)
Proficiency with OASIS documentation and Start of Care (SOC) processes
Experience with EPIC or similar EMR systems (electronic medical records)
Strong clinical skills in wound care, IV therapy, and chronic disease management
Excellent communication, patient education, and organizational abilities

Home Health RN Responsibilities

Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge OASIS assessments
Develop, implement, and manage individualized plans of care for home health patients
Provide skilled nursing care in patients' homes including wound care and IV therapy
Manage chronic disease care and medication education for patients and caregivers
Document patient visits and clinical care accurately in EPIC (Remote Client)
Collaborate with interdisciplinary team members (PT, OT, social work) to ensure coordinated homecare
