• Location: Montour Falls, NY 14865 — Finger Lakes area
• Facility Type / Setting: Skilled Nursing Facility (SNF Unit 3) — long-term care
• Contract Duration: 13 weeks (March 31, 2026 – June 30, 2026)
• Shift: Evening shift, 8 hours per shift
• Guaranteed Hours: 32 hours per week
• Estimated Weekly Pay: $1500-$1580/week
• Employment Type: Travel LPN / Travel Nursing
• Weekend/Holiday: Every other weekend and every other holiday required
• Breaks: Mandatory lunch break every shift

Job Requirements

• Active LPN / LVN license (must be valid for practice in New York or eligible for NY licensure)
• Minimum 2 years of recent experience in a long-term care (LTC) or skilled nursing facility (SNF) setting
• Proven skills in medication administration, resident assessments, and documentation
• Ability and willingness to float to other SNF units as needed
• Strong attendance, punctuality, and teamwork required

Primary Responsibilities

• Provide compassionate, high-quality nursing care to residents in the SNF Unit (resident care, LTC)
• Administer medications and treatments accurately and safely as prescribed
• Monitor, assess, and document resident health status and changes in the electronic health record (EHR)
• Participate in care planning, resident assessments, and interdisciplinary team meetings to ensure coordinated care
• Maintain compliance with facility policies, state regulations, and long-term care standards
• Float to other skilled nursing units as needed to support staffing and continuity of care
