Required Qualifications

• Minimum two (2) years of recent hospital pharmacy or inpatient pharmacist experience
• Successful completion of all educational requirements as mandated for state licensure (PharmD or equivalent as required by state)
• Current State Pharmacist licensure (Ohio licensure required or eligible)
• BLS (if required by state)
• EPIC experience preferred — experience with hospital EHR and medication management systems

Key Responsibilities (Clinical / Operational)

• Maintain adequate drug supply and ensure timely procurement to support inpatient pharmacy operations
• Prepare, sterilize, and label parenteral medications and IV solutions following sterile compounding policies
• Fill and dispense medication orders, including STAT orders, while ensuring accuracy and timeliness
• Document and record dispensed drugs; maintain pharmacy records and ensure regulatory accountability
• Perform monthly inspections of patient care units and pharmacy areas to maintain safety and compliance
• Control and document controlled substances in accordance with hospital and state regulations
• Participate in hospital formulary development, medication use evaluations, and Pharmacy & Therapeutics Committee activities
• Provide medication reconciliation and counseling for discharge medication lists to patients and families
• Monitor drug therapy regimens for contraindications, interactions, dosing accuracy, and appropriateness
• Support antimicrobial stewardship initiatives and pharmacokinetic dosing management as part of the clinical pharmacy team
• Contribute to quality assurance programs and support patient and employee safety policies and procedures
• Maintain confidentiality, cleanliness, and order within the pharmacy environment
• Perform other duties as assigned to support pharmacy and hospital operations
