Job Requirements / Qualifications
- Minimum two (2) years of bedside clinical experience required (LTAC, acute care, or related nursing background preferred)
- Active Registered Nurse (RN) license (state of practice or Compact RN where applicable)
- BLS Certification (required prior to first shift)
- ACLS Certification (required prior to first shift)
- Strong patient assessment, care planning, and clinical communication skills
- Prior experience with interdisciplinary team care, discharge planning, and rehabilitation services is a plus
Primary Responsibilities (LTAC RN / Night Shift)
- Perform comprehensive nursing assessments within 8 hours of patient admission and document findings promptly
- Develop individualized care plans and identify patient teaching needs and rehabilitation goals
- Provide direct patient care using the nursing process: assessment, planning, implementation, and evaluation
- Accurately document all assessments, interventions, and patient responses in the medical record
- Participate in patient rehabilitation programs and interdisciplinary team conferences to ensure coordinated care
- Initiate discharge planning and patient teaching beginning at admission
- Communicate changes in patient status to the care team to maintain continuity and patient safety
- Supervise and provide direction to ancillary nursing personnel and support staff
- Promote patient and employee safety by adhering to hospital policies, LTAC protocols, and regulatory standards
- Contribute to unit team building and foster a supportive work environment on the night shift
- Assume charge nurse responsibilities as needed after orientation
- Engage in ongoing professional development and continuing education to maintain clinical competencies
