Job Requirements / Qualifications

  • Minimum two (2) years of bedside clinical experience required (LTAC, acute care, or related nursing background preferred)
  • Active Registered Nurse (RN) license (state of practice or Compact RN where applicable)
  • BLS Certification (required prior to first shift)
  • ACLS Certification (required prior to first shift)
  • Strong patient assessment, care planning, and clinical communication skills
  • Prior experience with interdisciplinary team care, discharge planning, and rehabilitation services is a plus

Primary Responsibilities (LTAC RN / Night Shift)

  • Perform comprehensive nursing assessments within 8 hours of patient admission and document findings promptly
  • Develop individualized care plans and identify patient teaching needs and rehabilitation goals
  • Provide direct patient care using the nursing process: assessment, planning, implementation, and evaluation
  • Accurately document all assessments, interventions, and patient responses in the medical record
  • Participate in patient rehabilitation programs and interdisciplinary team conferences to ensure coordinated care
  • Initiate discharge planning and patient teaching beginning at admission
  • Communicate changes in patient status to the care team to maintain continuity and patient safety
  • Supervise and provide direction to ancillary nursing personnel and support staff
  • Promote patient and employee safety by adhering to hospital policies, LTAC protocols, and regulatory standards
  • Contribute to unit team building and foster a supportive work environment on the night shift
  • Assume charge nurse responsibilities as needed after orientation
  • Engage in ongoing professional development and continuing education to maintain clinical competencies
