- Weekly Estimated Pay: $2,390 – $2,520

- Assignment Duration: 13 weeks (Travel Contract)
- Shift: Day shifts, Monday–Friday, 8:00 AM – 5:00 PM (8-hour days)
- Hours per Week: 40
- Employment Type: Travel Contract (Home Health RN / Travel RN)
- Patient Load: Average 5–8 patients/day
- Productivity Expectation: 30 points/week
- Dress Code: Scrubs
- EMR: HomeCare HomeBase (HCHB)
- OASIS proficiency required
- Guaranteed Hours: No (opportunity to float to nearby clinics if needed)
- Weekend/On-Call: May vary; discuss during interview
- Local candidates accepted
Required Qualifications
- Minimum 1 year of recent Adult Home Health or Hospice RN experience (Home Health Nurse or Hospice RN)
- Active RN license in Oregon (must have license in hand)
- BLS/CPR certification required
- Proficient in HomeCare HomeBase (HCHB) EMR and OASIS (used within last 6 months or willing to complete retraining)
- Reliable vehicle, valid driver’s license, and agency-required liability insurance
- Comfortable with travel up to 75 miles from home clinic (travel RN / home health travel)
- Ability to complete documentation within 24 hours of patient visit
- Open to on-call rotations and accepting all scheduled patients
Primary Responsibilities
- Coordinate and direct home nursing care according to physician orders and agency policies
- Conduct home visits for patient care, including Start of Care (SOC), Wound Care, Follow-up Visits, and Recertifications
- Supervise and evaluate Home Health Aides and LPNs/LVNs
- Perform admission assessments, case management, recertification, and discharge planning
- Provide wound care, medication management, pain assessment, and symptom management
- Manage IVs, catheters, feeding tubes, and other clinical procedures as needed
- Ensure timely and accurate documentation using HomeCare HomeBase and OASIS (clinical documentation, point tracking)
- Participate in periodic office meetings and collaborate with the interdisciplinary care team
- Maintain infection control precautions and adhere to agency protocols
