Qualifications & Job Requirements

• Current, active Registered Nurse (RN) license in Massachusetts (required)
• CPR certification (required)
• Minimum 1 year of nursing experience required (or 6 months with BSN)
• Successful passage of pre-hire medication test
• Home care, home health, and IV therapy experience preferred
• Proficient computer skills and electronic health record (EHR) documentation
• BSN preferred for Home Health RN / Travel Nurse roles

Key Responsibilities

• Perform comprehensive nursing assessments at patient admission, evaluating physiological, psycho-social, environmental factors, and health history
• Integrate assessment data to identify problems and develop individualized home care plans consistent with case management objectives
• Utilize HHRG values to establish visit frequency and interdisciplinary utilization (home health case management)
• Refer patients to community resources and coordinate care with community providers
• Evaluate clinical needs; initiate, develop, implement, and revise patient care plans as needed
• Participate in interdisciplinary patient case conferences and evaluate effectiveness of care plans
• Supervise and review Home Health Aide plans of care every 14 days
• Notify physicians and care providers of significant changes in patient condition and obtain/clarify MD orders for all care delivered
• Complete admissions, discharges, and required home health paperwork in a timely, compliant manner
• Document skilled care clearly and concisely, including teaching, direct care, and patient/caregiver responses
• Plan and coordinate patient discharges and community follow-up when appropriate
• Perform other duties as assigned to support the home health agency and positive patient outcomes
