Job Requirements

• Active Registered Nurse (RN) license in the state of practice
• BLS Certification required prior to first shift
• ACLS Certification required prior to first shift
• Minimum 1 year of bedside clinical experience required (2 years highly desired)
• Experience in LTAC, sub-acute, or surgical settings preferred
• Strong clinical assessment, documentation, and interdisciplinary communication skills

Responsibilities

• Perform comprehensive nursing assessments within 8 hours of patient admission as an LTAC RN
• Develop individualized care plans in collaboration with patients, families, and interdisciplinary teams
• Provide direct patient care using the nursing process: assessment, planning, implementation, and evaluation
• Initiate and coordinate patient teaching and discharge planning from admission to support rehabilitation goals
• Document all assessments, interventions, and patient responses accurately and in a timely manner
• Promote patient and employee safety, adhering to hospital safety policies and procedures
• Supervise and provide direction to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees
• Communicate changes in patient status to care team members to ensure continuity and safety
• Participate in team conferences and rehabilitation programs to support patient recovery
• Contribute to a positive and efficient work environment through teamwork and professional development
