- Location: Claremont, NH 03743 (Claremont, New Hampshire; New England / near Boston, MA)
- Assignment Duration: 13 weeks (April 21, 2026 – July 21, 2026)
- Shift: 8-hour evenings (3pm–11pm), every other weekend required
- Hours per Week: 40
- Employment Type: Travel / Contract LPN
- On-Site: Yes
Job Requirements / Qualifications
- Active LPN license in New Hampshire (required)
- BLS certification (AHA)
- IV certification (or willingness to obtain during onboarding)
- COVID-19 vaccination required (declinations/exemptions not accepted)
- Minimum 3 years of recent LPN experience (within the last 6 months)
- Willingness to float to other units where experienced (LTC/SNF)
- Previous travel nursing experience highly preferred
- Rural / small facility experience highly preferred
- Must have own transportation (rural location; limited housing/public transit)
- Locals residing within 50 miles of the facility are not eligible
- All requested time off must be submitted with application and cannot exceed 5 days (holidays not approved)
- Submission requirements: Resume, skills checklist (within last year), two supervisory references from last two years, active certifications and license
Responsibilities – Long Term Care LPN / LTC Nursing Duties
- Continuously observe residents and document changes in mental and physical status
- Notify Supervisor and discuss physician notification as needed
- Give and receive detailed resident status reports at shift change
- Accurately receive, transcribe, and carry out physician orders
- Administer medications and treatments, document via electronic MARs/TARs
- Order and maintain pharmaceuticals for assigned units
- Supervise LNAs and assign nursing tasks in accordance with licensing and scope of practice
- Assist direct-care staff with nursing care and mentoring as needed
- Accompany physicians on resident rounds and facilitate communication between clinical team and families
- Administer oxygen and gastrostomy/jejunostomy feedings per facility guidelines
- Perform wound care and demonstrate knowledge of wound vacs
- Monitor infection control practices and ensure unit cleanliness
- Admit, discharge, and transfer residents, completing documentation and required notifications
- Complete 24-hour report for assigned unit(s) and maintain communication with Nurse Manager / Supervisor
- Communicate with residents’ families regarding concerns, new orders, and consents
- Manage assigned unit(s) during shift under direction of Nursing Supervisor or DON
- Follow State and Federal regulations regarding controlled medications
- Assist with orientation and continuing education of nursing staff and recommend disciplinary action for assigned staff as needed
