Job Requirements / Qualifications

• Active Occupational Therapy (OT) license in Wisconsin (WI) or compact-eligible license
• Minimum 12 months of Home Health experience within the past 3 years (home health occupational therapist)
• At least 1 year of overall Occupational Therapy experience required (hospital experience preferred)
• Experience with Homecare Homebase (HCHB) or similar EMR preferred
• Strong clinical assessment skills, ADL training, mobility and therapeutic activity planning
• Ability to work independently in the field and collaborate with multidisciplinary teams

Responsibilities

• Provide comprehensive occupational therapy services to patients in hospital and home health settings
• Conduct functional assessments, develop individualized treatment plans, and implement therapeutic interventions
• Deliver ADL training, adaptive equipment recommendations, mobility training, and fall prevention strategies
• Collaborate with nurses, PTs, case managers, and physicians to optimize patient outcomes and support discharge planning
• Document patient evaluations, progress notes, and care plans accurately in the EMR (HCHB preferred)
• Educate patients and families on therapy techniques, home safety, and strategies for independent daily living
