Job Requirements / Qualifications

  • Minimum 1 year of recent adult Home Health Occupational Therapy (OT) experience required
  • Valid Occupational Therapist license in Georgia (OT license required and must be in hand at submission)
  • Current CPR/BLS certification
  • Reliable vehicle, current driver's license, and agency-required liability insurance for travel OT roles
  • Proficiency with HomeCare HomeBase EMR and OASIS documentation (recent experience preferred; retraining available if needed)
  • Comfortable driving up to 75 miles from the home clinic and between patient homes (Home Health travel duties)
  • Ability to complete all documentation within 24 hours of patient appointment (timely OASIS/charting required)
  • Willingness to participate in on-call rotations and accept scheduled patients to meet productivity expectations

Responsibilities — Home Health Occupational Therapist

  • Conduct home visits for adult patients, delivering occupational therapy assessments, treatment planning, and interventions tailored to home care needs
  • Manage a daily caseload of 5–10 patients (targeting 6–8 productivity points per day; approximately 30 points per week)
  • Perform Start of Care, Follow-Up, and other skilled OT visits in accordance with agency protocols
  • Complete accurate and timely OASIS and HomeCare HomeBase documentation within required timeframes
  • Travel safely and efficiently between patient homes within the designated territory
  • Collaborate with nurses, PTs, SLPs, case managers, and interdisciplinary team members to optimize patient outcomes
  • Participate in periodic office meetings at the Covington clinic and follow agency policies, productivity expectations, and compliance standards
